Our team of consultants utilizes a comprehensive System Discovery & Analysis to identify opportunities for improvement as well as weaknesses or omissions in quality and/or customary offerings.
Following the completion of your System Discovery & Analysis, you’ll receive our Proposal & Estimate, which includes services provided by our in-house consultants, as well as education, tools, and services provided by our partners.
Fee structure is dependent upon the size of your organization, the number of offerings accepted in your customized plan, and the amount of consultation required for successful implementation. If your plan includes multiple components, we offer generous discounts for bundling services, tools, and education.

Our offerings rely on concurrent in-house consulting services to optimize delivery and successful implementation. Consulting services are available in several options and amounts based on your needs and preferences and are provided via Phone, Web, and In-person.

Service Line Implementation & Optimization
We have extensive experience planning and implementing customized and comprehensive service lines for cancer rehab, palliative services, and hospice care.
Tools and materials are developed and customized for each unique organization and integrate with existing resources. We drive the standardization of a screening-reviewing-referring process on the provider side.

Survivorship Services
Many elements make up Survivorship Services. Each brings awareness and improved acceptance of Survivors and the Survivorship journey to providers, patients, caregivers, and communities. Survivorship services improve the quality of life for survivors and caregivers in and around the organizations with which we collaborate. They also increase the collective health and well-being of the community by increasing overall engagement and participation.
We develop and promote new, or improve existing survivorship services that include, but are not limited to: wellness, fitness, sexual health, nutrition, finances, transportation, spirituality, emotional well-being, lifestyle, and employment.
Survivorship Care Planning
We provide education and direction regarding existing and evolving national requirements for survivorship care planning. We utilize Survivorship Care Plan (SCP) examples, trending and templates as well as customized formation of SCPs to assist your organization with building a sustainable SCP process.
National Accreditation and Certification Support
We educate healthcare systems about existing and evolving requirements of National Organizations and Accreditors and we can facilitate accreditation survey readiness and preparedness programs to prepare for surveys.
We drive the implementation of Quality Improvement initiatives for patient access, quality care, and value programming.

Tools & Materials
We create customized clinical and operational tools and materials used in creating, funneling, screening, measuring, reviewing, standardizing, retaining, marketing or optimizing patient services. The tools and materials align with current institution or organization resources and eliminate redundancies to improve communication within teams and with survivors.
Data Analysis & Benchmarking
Many years of direct patient care has provided us with first-hand knowledge utilizing patient functional outcomes measures. As such, we recognize the challenges and disruptions associated with data collection during patient care. However, we also know that data collection and benchmarking has become a required element in ensuring the delivery of best practices.
We provide our clients with guidance and direction on the most efficient and effective data collection strategies, including the development of customized tools and materials.
Once administration and collection protocols are in place and effective, we aggregate and analyze data to develop facility-specific goals, marketing materials, and process improvement strategies. Further, benchmarking of facility and patient data with other institutions provides a larger and more detailed picture that is shared with our clients.

We provide Survivors with support, education and workplace modifications to assist employees returning to work following or during illness.

Lifestyle Modification and Re-Integration
We provide survivors and caregivers with support, strategies, education, and home modifications to assist with and promote resumption of home life and day-to-day activities following or during illness.
