PURPOSE: We are a consultancy firm dedicated to increasing the existence, awareness, and access to survivorship services within healthcare organizations and the community.
SURVIVORS: are people with a chronic illness (including, but not limited to cancer) as well as the millions of others involved in patient care. Anyone touched by illness is a Survivor, including patients, caregivers, providers, and community members.
SURVIVORSHIP: is living with, through, or beyond a chronic illnesses or cancer diagnosis. Survivorship begins at diagnosis and continues throughout life. We are all on our own unique survivorship journey.
SURVIVORSHIP SERVICES: include supplementary services and support for Survivors that occur before, during, or following their primary treatments.
Services may include: cancer rehabilitation, palliative care, hospice, wellness programs, financial assistance, insurance counseling, transportation services, nutritional recommendations, psychosocial and/or counseling services, support groups, and many more.
Survivorship services improve the QUALITY of LIFE for Survivors. The more survivorship services that exist and are accessible in healthcare organizations and within the community, the higher the QoL for all Survivors!
VISION: Our work is guided by a shared vision that Survivors deserve timely, skilled, and affordable services that address the mind, body, and spirit, and are coordinated among their healthcare team, caregivers, employers, and community.
